As a Facial Plastic Surgeon in Beverly Hills, I have had the opportunity to take care of many people who wish to enhance their facial structure, and oftentimes even take on characteristics of those of the opposite sex. People who are considered to have attractive faces have several things in common. One of the most important facial features that is a constant in all attractive faces is strong bone structure, otherwise known as chiseled features that include the cheeks, chin, and jaw. This is common in both men and women; although there are some minor but important differences between the two sexes, this feature is desired by nearly all.
The most common wish that I hear from women is “Doc, I wish my neck and jawline was more defined and my face less saggy.” When asked to bring in photos of who they wish to look like it is always an actor or model with strong bone structure. Strong bone structure leads to good definition of the jaw line. This is the most important facial characteristic and the one responsible for making a person attractive enough to earn a living due to his/her appearance. I have had many female patients bring in photos of women’s faces that are striking and modelesque; however, it turns out that these are photos of transgendered men. I have seen them on talk shows and am amazed at how attractive their faces are. When I really think about it, it makes sense that they have a stronger bone structure than most women and consequently have striking features. This strong bone structure on a face with other feminine features often creates an exotic and beautiful appearance. A long, thin neck is the other feature most desired by my patients. This trait is also an extremely important facial characteristic that is a constant with all attractive faces. There are many ways to create or enhance these features for a person.
There are many options to enhance definition for the midface or cheeks and I have experience with all of them, including fat grafting, fillers, cheeklifts, facelifts, and cheek implants. Cheek implants are the option I use most because they give the best structure and definition and the results are permanent. All of the other options are short lived or temporary. When done correctly, cheek implants give the best results; however, this procedure can be tricky to learn and perform well. There are many different styles and shapes of implant – some better for women and others better for men – but there is some crossover. When a particular woman has very weak midface structure and wants a dramatic change, I will use the implant style that I use on men in that particular patient.
For the chin, there are also many options to enhance definition. In my opinion, there is really only one good option and that is a chin implant. Chin implants give better definition to the chin than any other technique and the results are permanent. The other options include fat injections and fillers, which give much less definition than chin implants and the results are temporary. There are many different styles of chin implant – some are used for women and others on men – there is only the rare circumstance that I will use the male version on a woman. It should be noted that people with severe deficiency of the chin may undergo an aggressive procedure where the chin bone is broken and advanced forward; however, in my opinion, it is rare for a person to benefit from this over a chin implant.
For the jaw there are many options to enhance definition and only two that make sense to me. The first option that I routinely use is the filler Radiesse. This filler can be perfectly molded to create a defined jaw angle and the result lasts about a year. The other option that I think is beneficial is a jaw implant. This procedure is very difficult to perform correctly and I most often recommend a custom implant. I will sculpt the implant from scratch to be tailor made to my patient’s anatomy. The jaw implants are more expensive than the filler but the results are permanent.
I have seen an increase in the demand for facial implants in my female patients over the last eight to 10 years and I think that this is because I am a strong believer in the benefits of enhancing the facial structure to maximize the beauty of the female face. With the use of the appropriate facial implants, I am able to achieve results that I believe are unattainable with any other technique.
Regarding the neck, there are also many options to create a more attractive appearance. The one that will suit someone will depend on that person’s anatomy. Some people will benefit from liposuction, a neck lift, or a lower facelift. The standard techniques of neck tightening are often very limited for many patients with thick necks. The technique that I have been honing for the last five years is significantly more powerful in sculpting and defining the neck than the standard Platysmaplasty technique. The subplatysmaplasty procedure is aimed at clearing out the thick fatty layer beneath the neck muscle (platysma), which the standard Platysmaplasty does not address. When this layer is removed, it allows the neck skin to drape nicely over the underlying reconstructed muscle corset that has been constructed. This allows the submentum (area under the chin) to be flatter and also for a deeper angle where the chin meets the neck – an attractive and desirable feature. I use this procedure in concert with facial implants and even facelifts for patients who wish to have the best facial aesthetic.
